Job Description

Certification Nursing Assistant/Home Health Aide

The Home Health Aide is a paraprofessional of the health care team wo works directly under the supervision of a Registered Nurse (RN), Supervisor of Services/Service Coordinator and perform various services for the client the majority of which are Personal Care or Activity of Daily Living. The Home Health Aide is assigned to assist the client in the manner that will promote a better quality of life and a safe environment for the client. 

Qualifications

Must be at least 18 years of age and preferable a high school graduate. Must meet all state and federal regulatory laws regarding criminal backgrounds, child abuse and sexual offender clearance, and have a minimum of two (2) years of relevant experiences as a Home Health Aide (preferably in a home care setting and under the supervision of a Registered Nurse (RN).


Must have successfully completed 80 hours of Home Health Aide Training and/or completed the Home Health Aide Skills Assessment Test with score of 80% percent or better. Should possess and maintain current certification in cardiopulmonary resuscitation. 


Must display good emotional health and is able to tolerate much standing, bending, stooping, and heavy lifting. Must be able to read, follow written instructions, and document care given. 


Must have had at least three (3) assignments and a cooperative attitude toward providing services.



Must meet all company requirements including pre-employment requirements.

Duties:

  1. Perform only under the supervision of an RN who has provided written instruction for patient care.
  2. Performs total care or assist client in all activities of daily living/personal care, such as, but limited to bathing, grooming, linen changes, and so forth.
  3. Assist client with transfer, ambulation, and range of motion exercises as assigned.
  4. Prepare nutritious meals with the client’s, adhere to client’s dietary restrictions, assist client with eating when necessary.
  5. Perform light housekeeping chores, which facilitates client’s self-care in the home setting.
  6. Assist client to the bathroom or with use of the bedpan.
  7. Respond to the client’s needs and attend to their request promptly
  8. Meet the safety needs of the client and use equipment safely and properly (e.g., bedrails and assistive devices).
  9. Provides proper care and observation of the client’s skin to prevent breakdown of tissue over bony prominence.
  10. Observe general conditions and report changes to the Nurse Supervisor.
  11. Complete records accurate and carries out assignments as requested.
  12. Must meet all state and federal regulatory laws regarding criminal backgrounds, child abuse and sexual offender clearances, drug testing.

Job Limitation

  1. May not administer medication or pre-fill insulin syringes unless state regulation allows such with special skilled training and certification.
  2. May not perform any sterile procedures or any intravenous procedures
  3. May not perform any procedures requiring the knowledge, training and skill of a licensed nurse.

Work

Certified Nursing Assistant/Home Health Aide will work an average of 28 to 40 hour a week, Monday thru Sunday. Home Health Aide will be assigned clients based on their skills and training. CNA/Home Health Aides will be expected to work, at least one weekend every month and work a very flexible schedule which includes early morning and evening hours.

Supervisor of Services/Client Service Coordinator

Job Summary:

The Supervisor of Services is a part of the administrative staff in the company. They are chiefly responsible for coordinating and staffing services for the clients which involves communicating with the directly with the clients, Home Health Aides and the nursing department. The Supervisor of Services is also responsible for the onboarding of new hires; performance evaluations and corrective plan of actions for the Home Health Aides.

Qualifications

Must be at least 18 years of age and preferably a college graduate with experience in the health care industry, social work, or some college related experience.


Must meet all state and federal regulatory laws regarding criminal backgrounds, child abuse and sexual offender clearances and drug testing.


Must have at least 1 year of managerial and supervisory experience and/or at least 1 year of knowledge about scheduling and staffing.


Must have effective communication written and verbal skills; possess the ability to guide, instruct and problem solve client’s and employee’s disputes.


Must be computer literate.

Duties:

  1. Receive all referrals and inquires for the services for the company
  2. Interview, screen, and test all applicants, make an initial determination as to rejection of applicant or completion of the hiring procedures then forward this information to the Director.
  3. Provide initial orientation to health paraprofessionals team
  4. Complete personnel folder on all employees, perform a monthly audit of the files – (non-administrative personnel.)
  5. Collect and photocopies licenses, certifications, identification card(s) for all potential hires. 
  6. Verifies licenses status with appropriate licensing entities, when appropriate and necessary.
  7. “Activate” each employ for each department (personnel, staffing, training, and payroll).
  8. Notify the Nursing Department of all new referrals
  9. Staff all assignments from the various referring agencies, company, or private referrals.
  10. Perform on-call duties to ensure staff coverage for clients
  11. Keep scheduling system updated, including availability, Time Activity Sheets, authorizations, deceased notifications, and schedule changes.
  12. Respond promptly and courteously to all client inquiries or problems
  13. Communicate continually with employee and client to evaluate services
  14. Assist in reassessment on Home Health Aide’s evaluation
  15. Audit, on a weekly basis, Home Health Aide’s Time Activity Sheets for accuracy and current payment.
  16. Serves as liaison between the caregiver’s Time Activity Sheets for accuracy and correct payment
  17. Perform all other duties assigned by the Director

Work Schedule:

Supervisor of Services can be a part time or full- time position, averaging no less than 28 hours weekly – Monday thru Friday. The Supervisor of Services will devote 100% of his/her time to the program. Supervisor of Services will provide weekend and evening coverage for the On-call Supervisor.

Nurse Manager

Job Summary:

The Register Nurse (RN) is a professional member of the health care team who provides supervisory services to client in compliance with the Nurse Practice Act and within standards of clinical practices as defined by the American Nurses’ Association and the policies and procedures of the company. Must possess and maintain current certification in cardiopulmonary resuscitation. The RN is responsible for supervising and monitoring of services and plan of care for the client.

Qualifications

Graduate of an accredited school of nursing and currently licensed in the state of Delaware.


Must have minimum of one (1) year of experience in a hospital, nursing facility, or in an acute care agency as a staff nurse and a minimum of six (6) months of experience and/or additional training for Home Health Care.


Must have successfully completed the Nursing Skill Assessment and Medication Test with a score of 75% to mastery.


Must display good emotional health and be able to tolerate physically much standing, stooping, bending, and heavy lifting and must comply with all company regulations.


Must be self-directed and able to work with little supervision and have good observational/nursing judgement, written and communication skills.


Must have effective communication written and verbal skills; possess the ability to guide, instruct and problem solve client’s and employee’s disputes.


Must be computer literate.

Work Schedule:

This position is paid per-diem. Cases are assigned based on zip codes and their geographic location. The Nurse Manager is responsible for bi-monthly reviews, post hospital/rehab visit, interim care plan developments, and staff training and re-education of Home Health Aide, when necessary.

Qualifications
